Welcome to WYSO’s Absence Form. We have provided an online process for any student who will miss an Orchestra, Ensemble and/or Group Class. All required fields are indicated with an * (asterisk) symbol. Call WYSO at 608-733-6283 if you have any questions.

Reminder for Orchestra Members: You have 2 allowed absences per concert cycle. You may have one additional allowed absence for a required school music activity. Absences exceeding two rehearsals in a concert cycle will prevent members from playing in that concert.
